Mobile Food Service or Retail Food Establishment
To acquire a Food Service License in the State of Ohio and within Butler County, you must fill out the following forms and applications. These applications are returned to the Butler County General Health District by mail, sent to the address on the right side of the page.

- Apply for Temporary Food Service License
- Apply for a Mobile Food Service or Retail Food Establishment License
- Ohio Food Safety Certification
